Members of the Community Care Coalition of Chester County (C-5) will discuss the points which will make for a smooth transition across the health care field—from hospital to rehab to home—as well as admission to the hospital under “observation” status and its ramifications under Medicare.
The Community Care Coalition of Chester County (C-5) was formed on June 15, 2012 to improve communication between multiple providers of care during the transition of a resident from hospital to a rehabilitation facility and/or home. The C-5 vision is to improve the quality of life of the community residents during the transitions of care process.
Members of C-5 include local hospitals, skilled nursing facilities, physician practices, home health agencies, hospice organizations, personal care homes, adult day care, home care agencies and community partners.
